Type 2 Diabetes (T2D) has been on the rise across Scotland for the past decade, having a significant impact on the lives of individuals and health of the population as a whole.

Recognising that this is driven by a range of environmental, societal and individual factors, health and social care providers in the East of Scotland are joining forces with partners from the public, third and private sectors to advance change across the whole system.
