We are working together to reduce Type 2 Diabetes (T2D) across the East of Scotland.
T2D has been on the rise across Scotland for the past decade, having a significant impact on the lives of individuals and health of the population as a whole.
Recognising that this is driven by a range of environmental, societal and individual factors, health and social care providers in the East of Scotland are joining forces with partners from the public, third and private sectors to advance change across the whole system.
We have also been tasked by the Scottish Government, as one of three early adopter sites, with implementing the national Framework for the Prevention, Early Detection and Early Intervention of Type 2 Diabetes.
To do this we are delivering new and enhanced Adult Weight Management and Child Healthy Weight services, which meet the National Service Delivery Standards.
With plans to adopt a Whole Systems Approach (WSA) to diet and healthy weight across the regional partnership and as part of implementation of the Framework, we are also supporting the roll out of five local whole systems pilot projects.
Our work supports Scotland’s Diet and Healthy Weight Delivery Plan and Public Health Priority 6, along with plans at a local level.
There has been an increased role for Regional Planning Groups in Scotland to lead the collaboration between NHS Boards on the planning and delivery of services, following the publication of the National Health and Social Care Delivery Plan in late 2016.
An extensive and ambitious programme of work has been agreed in the East of Scotland to deliver a transformation agenda which will support improved patient outcomes, sustainability, efficiency, financial savings and workforce planning across the region. This is underpinned by five regional objectives:
Our Type 2 Diabetes programme is the first initiative under the Prevention/Better Health workstream and will help inform the approach and learning for subsequent initiatives.
